Electronic Athletic Information and Permission Form

Please read carefully and download attachments before completing the electronic form.
Completion of this form is binding and confirmation that both the student and parent understand and agree to abide by all their responsibilities, laid out in the attached student handbook.

In the case of an accident or serious illness, I request the school to contact me. If I am unable to be reached, I authorize the  school to call my physician indicated below and to follow their instructions. If it is impossible to contact my physician, the school may make whatever arrangements are necessary to care for the health of my daughter.

Permission to drive to local AWAY games or be driven by another individual. (Sophomores and Juniors may not drive.)

I give my daughter permission to be driven by other licensed drivers. (parents, coaches/faculty) to away games.
In the event of an unforeseen transportation issue or emergency, I give my daughter permission to be driven by a licensed student driver on the same team to local away games. 

I give my daughter permission to drive to and from local away games (Seniors Only).

Permission to be dismissed early from school.

On occasion some games, meets, matches,etc. may be scheduled with a start time that necessitates an early dismissal from school. Usually these occur around playoff time, at the end of the season. Section 1 assigns crossover games, which involves a longer distance and travel. By checking yes below, you grant permission for your daughter to be dismissed before the official end of the school day as permitted by the athletic department.

Permission for Treatment by Certified Athletic Trainer

This document is to grant permission for your child to be treated by the Ursuline School’s certified athletic trainer.

Athletic trainers (ATCs) as defined by the National Athletic Trainers Association are “highly qualified, multi-skilled health care professionals who collaborate with physicians to provide preventative services, emergency care, clinical diagnosis, therapeutic intervention and rehabilitation of injuries and medical conditions. Athletic trainers work under the direction of a physician as prescribed by state licensure statutes. Athletic trainers are sometimes confused with personal trainers. There is, however, a large difference in the education, skillset, job duties and patients of an athletic trainer and a personal trainer. The athletic training academic curriculum and clinical training follows the medical model. Athletic trainers must graduate from an accredited baccalaureate or master’s program, and 70% of ATs have a master’s degree.”

I understand that treatment will involve a comprehensive evaluation and possibly physical therapy and therapeutic modalities such as therapeutic ultrasound and electric stimulation. I release the Ursuline School, the treating ATC and physician from any liability. I understand and support the fact that my daughter must comply with the directions given by the school and outlined in the handbook.

Communication for Ursuline Athletics - SportsYou App


I give permission for my child to download, create a free account and utilize the SportsYou app solely for the purpose of Ursuline athletic mobile communication. I understand that all information contributed in the Ursuline athletic SportsYou app by my child must follow the code of conduct outlined in the Ursuline Student Athlete/Parent Handbook as well as the Ursuline School Student Handbook.
